Function of Position:

The College is a partner in a Health Professions Opportunity Grant project.  This is a community-wide initiative, led by Action for a Better Community, to engage participants in educational pathways that will lead them to sustainable careers in high-demand health care careers.  The Navigator will assist eligible participants at both the Damon City Campus and the Brighton Campus to achieve vocational and educational success in the health care field by accessing services designed to prepare them for post-secondary education and employment.  The Navigator will develop an Individualized Service Strategy based on each participant’s strengths and identified needs, and will maintain formal relationships with Navigators working in partner organizations.  The Navigator also will ensure that each participant receives ongoing coaching/mentoring to support a successful outcome.  The Navigator reports to the Dean of the Division of Science, Health and Business at MCC and works closely with the HPOG Project Director at ABC.

Examples of Duties & Responsibilities:

  • Ensures an intake and assessment is completed for all individuals interested in enrolling in the program
  • Maintains and regularly updates information for all participants within the PAGES database system for the HPOG grant
  • Maintains hard copy records of all individuals who apply to the program
  • Engages in outreach and recruitment activities to identify participants who can benefit from work force readiness services and verifies eligibility for service
  • Develops and maintains rapport and routine personal contacts, as outlined in the ROC-HPOG procedures, with each participant enrolled in the program
  • Develops working relationships with colleagues at the College in order to support participants in their academic programs
  • Develops working relationships and rapport with Navigators working at partner locations and with the Project Director working at ABC and attends meetings through ROC-HPOG that are applicable to all Navigators
  • Processes participant requests for emergency funding and tracks related expenditures
  • Coordinates services of existing programs to meet the needs of each participant, and develops an Individual Service Strategy focused on educational and/or employment goals which identify specific targeted outcomes
  • Monitors progress towards established goals and engages in problem solving activities as necessary to remove barriers toward progress
  • Provides, at a minimum, monthly face-to-face contact with each participant and maintains more frequent contact with service providers through phone, email, and written correspondence
  • Works with Career and Transfer Center to assist participants in developing job acquisition skills, including job search strategies, resume development, interview preparation, completion of job applications and general expectations for workplace dress and behavior
  • Establishes partnerships with other providers, including schools and community agencies, to increase access to additional resources
  • Advocates for supportive services as needed to ensure participants can have their basic needs met such as access to transportation, childcare services, and clothing (uniforms.)
  • Maintains complete and accurate records regarding each participant, including documents supporting eligibility and contemporaneous documentation of services provided
  • Prepares monthly reports as required by funding sources and/or management
  • Attends all required college and project specific training workshops and meetings
  • Works with Financial Aid to assist participants with navigating the financial requirements for enrollment in post-secondary education
  • Contributes to the College through Division, department and College committees
  • Performs other duties as assigned by supervisor

Required Qualifications:

  • Earned bachelor’s degree in social work, counseling, human services or closely related field from an accredited institution
  • Three years of experience in a related field or demonstrated knowledge and experience providing case management services
  • Ability to develop effective working relationships at all levels within and outside of the organization and to work as part of a team
  • Commitment to supporting an inclusive environment on campus. Experience working with diverse populations with a particular understanding of the issues relating to such populations
  • Experience in conducting community outreach needs assessments and service plans for program participants
  • Demonstrated knowledge of available supportive community resources to assist in information and referral services for program participants
  • Excellent interpersonal skills and ability to develop relationships with participants
  • Excellent verbal and written communication skills
  • Excellent organizational skills with attention to detail
  • Good understanding of issues facing individuals who are low-income and who are eager to move from dependence on public assistance to financial independence
  • Requires a valid Driver’s License and access to reliable transportation.

Preferred Qualifications:

  • Five years of experience in a related field or demonstrated knowledge and experience providing case management services
