ABOUT THE POSITION:  RSSP Facilitator conducts interviews with disabled Temporary Assistance Recipients and develops corresponding Restoration of Self Sufficiency Plan to assist clients in moving towards Self Sufficiency. Duties include supporting clients in overcoming obstacles to first appointment scheduling and monitoring documenting client compliance with RSSP requirements.


  • Interviews clients referred by Monroe County DHS who have been identified by DHS as having a medical recommendation for treatment of a physical or psychological medical condition.
  • Completes DHS required Mental Health Assessments and reviews all medical documentation provided.
  • Develops written Restoration of Self Sufficiency Plan (RSSP) with client based upon medical documentation and assessment information.
  • Refers to, and coordinates with, client’s MCO, Medical and mental health providers necessary to address current medical condition in accordance with RSSP.
  • Assesses for barriers to RSSP adherence and uses strength-based approach to develop strategies to overcome identified barriers.
  • Reviews needs of recipient to assess whether care coordination would be appropriate; makes those referrals as needed.
  • Schedules first medical and mental health appointments as designated by RSSP.
  • Monitors client attendance at scheduled appointments via phone calls, secure email correspondence or other established practices.
  • Data entry of appointments into tracking database.
  • Utilize Motivational Interviewing strategies and other evidence-based practices to encourage change and movement towards self sufficiency.
  • Maintains written client records, files and documentation, including, but not limited to, medical documentation and client consent forms for sharing of information as required by Monroe County DSS and state, federal and HIPAA confidentiality standards.
  • Maintains up to date medical forms to document client’s level of disability.
  • Performs other related duties as assigned.

  • Minimum two years direct experience in behavioral health field and or customer service, working with adults
  • Associate’s degree in a Human Service related field preferred
  • Experience in Vocational Rehab field a plus
