General Description

As a Licensed Practical Nurse (LPN) you will ensure high quality medical care and services for adults with intellectual and developmental disabilities by coordinating, scheduling, communicating and documenting medical appointments and issues. You will also transport and accompany participants to medical appointments for all health needs. Other responsibilities include acting as a liaison between residential and day services staff, dieticians, the medical team and community providers, administering medication, and maintaining accurate and complete documentation.

SIGN ON BONUS AVAILABLE:  $1000.00 sign on bonus ($500 upon completion of  6 months of successful employment, and $500 upon completion of one year of successful employment).

Position hours are: Daytime hours, 6a – 2:30p or 8a – 4:30p

Job Requirements

    • Basic Computer Literacy as evidenced by the ability to go online and conduct basic searches on the Internet, check his/her email account, create and send e-mail with an attachment and use simple email features.   Can open spreadsheets and word processing documents to read and make simple edits.
    • Excellent communication skills including both verbal and written communications for training and support purposes.
    • Strong organizational and prioritization skills.
    • Strong problem solving ability and attention to detail

Minimum Qualifications

  • Current New York State driver’s license.
  • Licensed by the NYS Department of Education as LPN. Experience in the field of intellectual and developmental disabilities preferred.
