JOB PURPOSE:

A registered nurse (RN) who is works collaboratively to identify and proactively manage the care needs of patients screening positive for symptoms of COVID 19.  The RN provides assessment, care coordination, advocacy and coaching for patients.

MAJOR RESPONSIBILITIES (Essential functions):

RESULT #1: Jordan Health patients receive high quality, consistent, competent, patient- centered care.

• Create a culturally competent atmosphere of patient team care respectful of a patient’s holistic needs (cultural, physical, psychosocial, etc.).

• Aid in improving patient customer service experience through communication with patients and families.

• Provide patient education on topics including COVID and COVID prevention.

• Utilize critical thinking skills to facilitate regular clinical operations and processes to ensure efficient flow of patients and ensure quality of care delivery.

• Independently practice at the top of scope.

• Collaborate with internal and external team members to more effectively coordinate care services for Jordan Health patients.

• Follow up with patients individually regarding the outcome of COVID 19 testing

• Work collaboratively with Employee Health Nurse when a patient test positive for COVID and was in the center

• Cross-cover for other Team Care RNs as needed/requested by Practice Manager

• Educates patient’s on the Patient Portal

RESULT #2: Jordan Health financial and regulatory objectives are supported.

• Increase program revenue by increasing daily visits to the Respiratory and Peds Acute Clinics department through supporting clinicians and improving flow of patients through clinics.

• Handle confidential information in accordance with HIPAA as well as state and federal privacy and confidentiality rules

• Document all activities in the Electronic Medical Record.

• Assistance in maintaining compliance with Joint Commission standards.

• Work with the QI department to monitor COVID 19 testing results done within the Jordan Health system and outside of the system

EDUCATION AND EXPERIENCE REQUIRED:

• 1 year of nursing experience required.  Preferred experience in urgent care, emergency department, or pediatrics

LICENSES AND CERTIFICATIONS:

• Valid RN licensure in NYS with no negative actions

• Current BLS/CPR

SPECIAL SKILLS, KNOWLEDGE REQUIRED:

• Strong computer skills

• Excellent customer service skills both phone and in person

• Ability to work well with others in a multi-disciplinary team

• Ability to organize and multi-task
